SERVICES PROVIDED

A residential support worker is on duty 24/7 to provide a high level of social support throughout the core hours of the day.

This will enable the service to meet the individual needs of the resident as identified in the care plan. Also, there is waking night staff during the nights.

General monitoring of individual adherence to agreed treatment plan, medication compliance, and ensuring individual resident attend their therapeutic sessions and outpatients appointment.

Liaison with relevant care professionals and other agencies.

Providing input to periodic reviews under the care programme approach.

Maintaining individual daily records, which will be made available to the appropriate care professionals.

To assist in developing independent living skills such as budgeting, cooking, ironing, etc.

Counselling and emotional support.

Advice and promote advocacy.

Arrange social events
